- Quest and Quest EAP
At this time Heather is not in network with medicaid plans.
Behavioral health (therapy) could be covered in full or in part by your health insurance. Please contact your health insurance provider to determine what services are covered by your plan. Or if your insurance isn’t listed above, ask if they accept superbills from out-of-network provideres. The number to call is typically found on the back of your insurance card.
The following questions may be helpful to ask your health insurance company about your benefits:
- Does my insurance plan include behavioral health benefits?
- What is my deductible and has it been met? When is my deductible period? Does my deductible apply to behavioral health?
- What is my copay?
- How many sessions per year are allowed?
- Are teletherapy sessions covered?
Out of Network
If your insurance was not in the list above, you may still be able to access your benefits. Receipts called superbills can be provided to clients wishing to use their out-of-network benefits. The client submits this document to the insurance company. Some plans may reimburse the client a portion of your costs listed on the superbill. Please reach out to your health insurance company before starting therapy to determine if & how much they would reimburse you for out of network behavioral health services.

90837 – 53-60 minutes appointments
90847 – family with client (couples or family sessions)
Self Pay
Rates for self-pay individual therapy services:
Initial Intake Appointment: $180.00
Therapy Session: $160.00

Payment
Fees are due at the time of service. Acceptable forms of payment are HSA/FSA cards, cash, check, and major credit cards. All clients are required to have a credit card on file in the client portal.
Cancellation Policy
If you miss or cancel an appointment with less than 24 hours notice, you will be charged a fee of $80.
